Information regarding Covid-19 vaccinations

A new location for group vaccinations will open near Kashiwa Station

In Kashiwa City, group vaccinations at the Chuo Taiikukan (Central Gymnasium) will end on Friday, August 27. Beginning in September, a new vaccination location will open at The Crest Hotel Kashiwa. In this issue, we will provide information regarding group vaccinations at the hotel.

Vaccination Schedule

Starting on Wednesday, September 1, on certain Wednesdays, Thursdays and Sundays from 9:00am to noon and 2:00pm to 5:00pm. (Closing date is to be determined)

2nd dose will be scheduled 4 weeks after the 1st dose, on the same day of the week and time slot.

Eligibility

Registered residents of Kashiwa City, age 12 and older who haven’t had their first vaccination yet.

About 300 people per day

Monday, August 23 at noon

Appointments for the following week will be accepted beginning on Mondays at noon.

▶Vaccination tickets have been sent to everyone who is 12 years old as of August 1, 2021. Vaccination tickets will be mailed in the month after their birthday to people who will be 12 years old on or after August 2.

How to get to The Crest Hotel Kashiwa

The hotel is a 5-minute walk from the Kashiwa Station West Exit.

There is no parking available at the hotel, so please use public transit.

Regarding vaccinations for people age 15 and under

People age 12 and older are eligible to get either the Pfizer or Takeda/Moderna vaccine. Before getting vaccinated, please make sure that both the individual to be vaccinated and their parent or guardian fully understand the vaccine side effects.

 

Vaccinations for people age 12 to 15

▶Have a parent or guardian fill in the "person receiving a vaccination or signed guardian" section on the pre-vaccination medical questionnaire

▶The person receiving the vaccination must be accompanied by a parent or guardian at the vaccination location.

The city is accepting applications for the issue of Covid-19 Vaccination Certificates (Vaccine Passport). Applications are processed, and certificates issued in about 1 to 2 weeks, so same-day issuance is not available. If you require a vaccination certificate, please apply well ahead of time.

People who are planning to travel overseas, and have been vaccinated using the vaccination tickets issued by Kashiwa City.

Where to get an application form

You can get an application form from the Kashiwa City website, the reception desk at the City Hall Main Building, Shonan Branch Office (Shonan Building, 1st Floor), or the Public Health Center General Affairs and Planning Division (Wellness Kashiwa 3rd Floor).

Application

Fill out the application form, and obtain a copy of your valid passport, vaccination certificate (vaccination record), and identification documents for the applicant (My Number Card, etc.). Mail the application form and document copies, along with a self-addressed envelope affixed with an 84 yen postage stamp, to 277-0004, Kashiwashita 65-1, Kashiwa City Public Health Center General Affairs and Planning Division.

Pick up your National Health Insurance or Latter-Stage Elderly Medical Insurance card

Health insurance cards were renewed on August 1, and the new cards were sent by registered mail in mid-July. The Kashiwa City Hall is keeping health insurance cards that were not delivered. If you have not received your health insurance card, please pick it up or have it re-sent as soon as possible.

 

How to get your health insurance card

▶At the reception counter = Bring official photo identification, and pick up your card in person at the Health Insurance and Pension Division (City Hall Main Building, 1st Floor)

▶To have the card re-sent = Contact the Health Insurance and Pension Division

Other

If you have social insurance or another kind of health insurance, you need to withdraw from National Health Insurance. To do this, please bring the current health insurance cards and National Health Insurance cards for each person who needs to withdraw, and complete the withdrawal application at the Health Insurance and Pension Division, Shonan Branch Office (Shonan Building, 1st Floor), any branch office, or the Kashiwa Station City Hall Service Center.

 

Temporary closure of the Night Emergency Medical Treatment Center in the Kashiwa City Medical Center

The Night Emergency Medical Treatment Center in the Kashiwa City Medical Center (Wellness Kashiwa) will be temporarily closed in order to open the Kashiwa City Covid-19 Night Vaccination Center.

During the period of closure, please use the hospitals on duty in Kashiwa City for medical emergencies at night. To find out which hospitals are on duty for each day, call the recorded information service (phone: 7163-0119) or check the Kashiwa City website.

Submit the income status report for Special Child Support Allowance

If you are receiving Special Child Support Allowance, please submit the income status report, along with your allowance certificate, by Monday, September 13 at the Disability Welfare Division (City Hall Annex, 2nd Floor) or the Shonan Branch Office (Shonan Building, 1st Floor). If you submit the report after the deadline, you will not be able to receive your allowance as scheduled.
