Notice regarding Covid-19 vaccinations

In Kashiwa City, vaccination tickets (coupon tickets) have been mailed to people age 65 and older. Vaccination tickets are now being prepared for mailing to people age 64 and under. We will provide information here about how to apply for vaccinations for people with underlying health conditions, and group vaccinations.

If you are 65 years of age or older, and have not received your vaccination ticket, please contact the Kashiwa City Covid-19 Call Center.

Applications for people age 64 and under with underlying health conditions will be received until Friday, July 9

For people with underlying health conditions who have made an application, vaccination tickets will be mailed in order that the applications were received. Vaccination tickets are scheduled to be mailed on Monday, July 5 to those who have made an application as of June 23.

▶Vaccination tickets are scheduled to be mailed on Monday, July 12 to those who apply between June 24 and Friday, July 9.

 

Where to find an application form

Application forms are available at the Public Health Center, Health Promotion Division, Covid-19 Vaccination Information Counter (Wellness Kashiwa, 3rd Floor), Kinrin Centers (not including Kashiwa Village, Hokubu, and Nedo), branch offices, etc.

Application

By Friday, July 9, please complete the required parts of the form, and mail it to 277-0004, Kashiwa-Shita 65-1, Kashiwa City Public Health Center, Health Promotion Division, Covid-19 Vaccine Information Counter (to be received by the date), or submit it in person. Applications can also be made through the Kashiwa City website.

An application is not required if any of the following items apply

An application is not required for people who have the following passbooks or are receiving the following support as of June 1. Vaccination tickets are scheduled to be mailed on Monday, July 5.

Changes have been made to part of the city’s group vaccinations

Regarding the city’s group vaccinations, changes have been made to the time period, the schedule, and the type of vaccine that is scheduled for use.

Central Gymnasium (Chuo Taiikukan)

Other locations are also being coordinated.

Time period

Friday, July 3 to Friday, September 24

The general schedule will be from Monday to Sunday, including public holidays. There may be changes to the schedule.

9:00am to noon, and 2:00pm to 5:00pm

Subject to change depending on the day of operations

Eligibility

Registered residents of Kashiwa City who have vaccination tickets.

Approximately 2,000 to 3,000 people per week

Type of vaccine scheduled for use

Takeda/Moderna

The type has been changed from the Pfizer vaccine due to the national vaccine supply situation.

Caution

Two vaccinations are required for both the city’s group vaccinations, and individual vaccinations provided by medical facilities in the city. You must receive both vaccinations at the same venue.

 

How to make an appointment for a group vaccination

When your vaccination ticket is delivered, and you wish to receive a group vaccination, please make an appointment using the dedicated phone number, or the online appointment site published on the Kashiwa City website.

 

3 things to have ready before your appointment

①Your vaccination ticket number

Please make sure you are ready with the 10-digit “ticket number” written on your ticket.

 

②Date of birth written in Western Calendar format

Please verify the date of birth in Western Calendar format of the person for whom you are making the appointment.

 

③Your preferred date and time for the first vaccination

Appointments for group vaccinations will be made for the second vaccination at the same time.

Important points to note

Cancelling vaccination appointments without permission and double-booking appointments can lead to the unnecessary disposal of vaccines.

If you need to cancel an appointment, make sure to do it well ahead of time. We need to ensure the efficient administration of vaccinations to as many people as possible.

 

How to make an appointment at the Large-Scale Vaccination Centers

People 65 years of age and older can receive vaccinations at the Chiba Prefecture Vaccination Center for Older Adults, and people 18 years of age and older can receive vaccinations at the Japan Self-Defense Force Large Scale Vaccination Center in Tokyo. People who have not received vaccination tickets, and would like to receive a vaccination may submit an application for a vaccination after checking the availability of appointments.

Special support benefits will be paid to child-raising households

Special Support Benefit Payments for Child-Raising Households will be paid to child-raising households that are experiencing financial difficulties due to the effects of Covid-19. Notices will be sent on Friday, July 2 and support benefits will be paid to households that are exempt from Resident’s Tax, and receive the April amount for Child Allowance and Special Child Dependent Allowance. Other households that are eligible may be able to receive the support benefit payment through an application.

Eligibility

Households with children born on or after April 2, 2003, or children with a disability who were born between April 1, 2001 and February 28, 2022, and meet any of the following criteria.

▶︎Exempt from Resident’s Tax for the 2021 fiscal year.

▶︎Households that experienced a sudden change in income due to the effects of Covid-19, and whose income is currently at the same level as a household exempt from Resident’s Tax.

Benefit payment amount

50,000 yen for each eligible child

Application

Application forms are available at the Children’s Welfare Division (City Hall Annex, 3rd Floor). Please complete an application, attach the required documents and mail it to 277-8505, Kashiwa City Hall Children’s Welfare Division, or submit the application in person. Applications must be received by Monday, February 28 of next year.

The applicable time period will be extended for Disability Allowance with National Health Insurance and Late-Stage Elderly Medical Insurance

With consideration to the Covid-19 situation, the applicable time period will be extended by 3 months for Disability Allowance through National Health Insurance and Late-Stage Elderly Medical Insurance.

Applicable period

January 1, 2020 to September 30, 2021

Eligibility

People who meet all of the following criteria.

▶Enrolled in Kashiwa City National Health Insurance or Chiba Prefecture Late-Stage Elderly Medical Insurance

▶Receiving salary payments, etc.

▶People who took 4 or more consecutive days leave to recover from Covid-19 or symptoms suspected to be Covid-19.

▶People whose salary was fully or partially unpaid.
