Group vaccinations for Covid-19 will begin

In Kashiwa City, we are working towards mailing vaccination tickets (coupon tickets) to all residents 65 years of age and older by Friday, June 25. For people who do not have a regular family doctor we will provide information on how to make an appointment for group vaccinations, and what to do on the day of your vaccination.

Group vaccinations will begin on Saturday, July 3

▶Time period: Saturday, July 3 to Thursday, September  30 (Generally scheduled for Wednesdays, Thursdays, Saturdays and Sundays, including public holidays.)

▶Time: 9:00am to noon, 1:30pm to 4:30pm

▶Location: Central Gymnasium (Chuo Taiikukan) (other locations are being evaluated)

▶Eligibility: Registered residents of Kashiwa City who have vaccination tickets

▶Capacity: Approximately 2,000 to 3,000 people per week

▶Cost: Free of charge

▶Type of vaccine scheduled for use: Pfizer

 

Appointments will be accepted starting on Monday, June 28

When your vaccination ticket is delivered, and you wish to receive a group vaccination, please make an appointment using the dedicated phone number, or the online appointment site published on the Kashiwa City website. The appointment time slots are every 20 minutes.

▶Items to check when making an appointment by phone

①The 10-digit ticket number written on the vaccination ticket, and your date of birth written in Western Calendar format.

A phone number where you can be reached during the daytime

Your preferred location for the group vaccination. Be prepared to choose your preferred vaccination date out of the possible appointment dates and times offered by the call center.

Please be aware that the phone line for appointments is expected to be extremely busy.

3 things to have ready before your appointment

①Your vaccination ticket number

The 10-digit “ticket number” written on your vaccination ticket is the unique vaccination ticket number assigned to each person.

②Date of birth written in Western Calendar format

Please verify the date of birth in Western Calendar format of the person for whom you are making the appointment.

③Your preferred date and time for the first vaccination

Appointments for group vaccinations will be made for the second vaccination at the same time. The date of your second vaccination will automatically be scheduled for the same day of the week, 3 weeks after your first vaccination date. Please be prepared to confirm your schedule while you are making the appointment.

 

Things to check before your vaccination

If you forget the required items on the day of your appointment, you may not be able to receive the vaccination. Please check that you have everything you need before heading out to the vaccination location on the day of your appointment.

 

Things to check before you leave your house

□ You are in good health.

□ You do not have a fever.

□ You have a mask.

□ Your Prevaccination Screening Questionnaire is filled out correctly with no omissions.

Please bring your “Kusuri Techo” (Prescription Record Booklet) if you are taking any medication. Be sure to fill in details if you have had severe allergy symptoms or become ill following any prior vaccinations.

 

Please note

If you have not filled out your form completely, you will not be able to proceed smoothly with the vaccination process.

 

Check that you have everything you need

□ Your vaccination ticket (coupon) should be intact. Do not tear it off.

□ Your completed Prevaccination Screening Questionnaire

□ Personal identification documents (My Number Card, health insurance card, drivers’ license, etc.)

□“Kusuri Techo” (Prescription Record Booklet) (if you have one)

 

Wear appropriate clothing on the day of your vaccination

The vaccine is injected into the muscle in the upper part of the arm close to the shoulder. On the day of your vaccination, please wear clothing that allows you to expose your shoulder easily.

 

Saturdays, Sundays and public holidays only! A free shuttle bus will be available

A free shuttle bus to and from the group vaccination site at the Central Gymnasium (Chuo Taiikukan) will be operated from Kashiwa Station and Kita-Kashiwa Station on Saturdays, Sundays and public holidays only. Parking spaces are limited, so please use the free shuttle bus or public transportation.

Other ways to get a vaccination

The Covid-19 vaccine can be obtained by methods other than the group vaccinations provided by the city. The appointment system, etc. is different for each vaccination method. For more information, please visit the Kashiwa City website or contact any of the other information resources.

Vaccination tickets are being prepared for mailing to people aged 16 to 64 with underlying illnesses

On June 1, the eligible age to receive the Covid-19 vaccination was lowered to 12, but Kashiwa City will first begin to accept applications for people age 16 to 64 with underlying illnesses. There are underlying illnesses which have a higher priority for vaccinations. For those wishing to receive a vaccination, vaccination tickets will be mailed out based on the priority identified in the application. If you are able to use the internet, please make an application through the Kashiwa City website.

 

Where to find an application form

Application forms are available at the Public Health Center, Health Promotion Division, Covid-19 Vaccination Information Counter (Wellness Kashiwa, 3rd Floor), Kinrin Centers (not including Kashiwa Village, Hokubu, and Nedo), branch offices, etc.

By Friday, July 9, please complete the required parts of the form, and mail it to 277-0004, Kashiwa-Shita 65-1, Kashiwa City Public Health Center, Health Promotion Division, Covid-19 Vaccine Information Counter (to be received by the date), or submit it in person. Applications can also be made through the Kashiwa City website.

Applications are being accepted for school financial assistance

Kashiwa City provides financial assistance for children attending school from households experiencing financial difficulties. Households that experienced a decrease in income as a result of COVID-19 may also be eligible, so please submit an application for financial assistance. Applications are accepted at any time.

 

Eligibility

Parents or guardians who are struggling financially with daily necessities, and whose child meets either of the following conditions during the period between April 1 and the date of the application.

▶Enrolled in a public elementary or junior high school in Kashiwa City

▶Residing in Kashiwa City while enrolled at a public elementary or junior high school in another city.

Not including those who can receive school financial assistance from another city.

 

Period of eligibility for financial assistance

From the month following the date that the application was received to March of next year.

 

Expenses covered by financial assistance

The cost of school supplies, school lunch fees, PTA membership fees, etc.

 

Application

Application forms are available at every elementary or junior high school, or from the Schools and Education Division (Shonan Building, 2nd Floor). Submit the completed application by mail, (addressed to 277-8503, Schools and Education Division), or in person at the school your child is attending, or at the Schools and Education Division.

Other

If income decreased significantly from January onwards, and the total income for this year is expected to be below the standard amount for approval, this year’s income situation will be considered in the evaluation

 

▶ Approval is determined based on the total income amount from the previous year.

If there was a significant drop in income from January, and the expected total income for this year falls within the approval criteria, the income situation for this year may be evaluated.

▶The income amount for meeting the approval criteria depends on factors including family structure, type of housing and total income amount.

Have you picked up your My Number Card?

If you applied for your My Number Card before the end of April, you will be able to receive Maina-Points worth up to 5000 yen after completing the necessary application. The number of appointment slots to pick up your card has been expanded at the Citizen’s division (Kashiwa City Hall Main Building, 1st Floor) until September, so please make an appointment if you have not picked up your My Number Card yet.

The school district has been decided for the new elementary school in the Kashiwa North Area, East District

In order to accommodate the growing number of children in the Kashiwa Tanaka Station area, and provide a school of adequate size, Kashiwa City is relocating the current Tanaka Kita Elementary School, and preparing to open a new elementary school in April 2022. The elementary school district for the new school will be communicated here.
