Covid-19 Vaccination Notice

The Covid-19 vaccine will be provided free of charge to those who wish to be vaccinated. In order to keep operations running smoothly at medical facilities, please wait until you have received your vaccination tickets (coupon tickets) before making an appointment.
Division of levels for Group 2
People who are eligible for Level 1
Those who are certified for nursing care and support and have been admitted to a nursing care facility.
People who are eligible for Level 2
・Those who are certified for nursing care and support and not included in Level 1.
・Those who have a passbook for a person with a physical disability, rehabilitation passbook, or health and welfare passbook for a person with a mental disability.
Mailing of vaccination tickets
Vaccination tickets have already been mailed to people in Level 1 and 2. People in Level 1 who wish to receive a vaccination should check with the facility where they have been admitted. People in Level 2 should make an appointment after referring to the list of medical facilities included with the vaccination tickets.

Vaccination ticket mailing period for people in Level 3
People who are not included in Level 1 and 2.
▶80 years of age and older: May 14
▶75 to 80 years of age: Scheduled for Friday, May 21
▶70 to 75 years of age: End of May to the beginning of June
▶65 to 70 years of age: Mid-June and later
The vaccination ticket mailing period may change depending on the vaccine supply and the status of appointments and vaccinations at the medical facilities.
Chiba Prefecture Covid-19 Countermeasures Cooperation Fund (Part 6)

A cooperation fund will be paid to business operators who cooperated with Chiba Prefecture’s request for implementation of measures to prevent the spread of Covid-19.
Eligibility
Restaurants that reduced hours of operation to 5:00am to 9:00pm (with alcohol served only between 11:00am and 8:00pm) between April 1 and April 19.
Payment amount
760,000 yen
Application period
By Friday, June 18

Payment of special support benefits for single-parent households

Special support benefits for living expenses will be paid to single-parent households that have experienced a decline in their financial situation as a result of Covid-19. The benefit will be paid on April 28 to recipients of the Child Dependent Allowance payment for April. Those who did not receive the Child Dependent Allowance will require an application.
Eligibility
Single-parent households that meet any of the following criteria:
①Public pension recipients whose income for 2019 was below the eligible limit to receive child dependent allowance.
②Those whose income decreased due to the effects of Covid-19, and is below the eligible limit to receive child dependent allowance.
Benefit payment amount
50,000 yen for each child
Distribution of application forms
Application forms are available at the Children’s Welfare Division (Kashiwa City Hall, Annex 3rd Floor), or they can be downloaded from the Kashiwa City website.
Application forms will be sent on Monday, May 17 to those approved for child dependent allowance, single-parent household medical fees subsidy, child-rearing allowance for orphaned children in Kashiwa City, etc.
Application
By Monday, February 28 of next year, complete the application form, attach all required documents, and mail it to 277-8505 Kashiwa City Hall, Children's Welfare Division (to be received by the deadline), or bring it directly to the Children's Welfare Division.
