Vaccination tickets (coupon tickets) are being sent by mail to adults aged 65 and older. In this special issue, we will explain the process for older adults to receive their vaccinations, and the required preparations.

The process for receiving a vaccination

The vaccination ticket (coupon ticket) will be delivered in a white envelope

Make an appointment at a medical facility

A document titled “Notice on Covid-19 Vaccinations” (新型コロナワクチン接種のお知らせ) will be included in the envelope with the vaccination tickets sent by the Kashiwa City Public Health Center. Use this document to check the list of medical facilities providing vaccinations, and make an appointment.

Fill out the Prevaccination Screening Questionnaire

Before you go to the medical facility to receive your vaccination, fill out the required parts of the “Prevaccination Screening Questionnaire for COVID-19 Vaccine” (予診票). Please leave out the sections to be completed at the medical facility. There are two “Prevaccination Screening Questionnaire for COVID-19 Vaccine” forms – one for each of the two doses of the vaccine. Please keep the forms in a safe place with the vaccination tickets, so that they are not lost.

The English titles of the documents in this special issue have been changed to match the translations provided on the Ministry of Health, Labour and Welfare website.)

Receive your vaccination at the medical facility at your appointment time

Check your body temperature before you leave the house. Once you have confirmed that you do not have a fever, please go to the medical facility for your appointment. The doctor will check your health condition at the time of your appointment, and determine whether or not you can receive the vaccination. If there is no problem, you can receive the vaccination.

What to bring

・Vaccine tickets (coupon tickets) – please do not cut them off

・One “Prevaccination Screening Questionnaire for COVID-19 Vaccine” (予診票) with the required parts completed.

・Personal identification documents (health insurance card, driver’s license, etc.)

・Prescription medicine passbook (if you have one)

Keep your vaccination certificate

After you receive your vaccination, you will receive a certificate. Make sure to keep your vaccination certificate in a safe place. The vaccination certificate will serve as proof that you have received a Covid-19 vaccination.

You must bring the vaccination certificate from your first vaccination when you go to the medical facility to receive your second dose of the vaccine.

Two doses of the vaccine are required

You will need to receive two doses of the same type of vaccine with a certain amount of time between doses in order for the vaccine to be effective. Regarding the appointment for your second vaccination, please consult with the medical facility where you receive your first vaccination.

Medical facilities are currently providing regular medical care as well as conducting vaccinations for medical workers and older adults at the same time.

For this reason, vaccination tickets (coupon tickets) are being mailed in stages to a number of people for whom it is possible to provide vaccinations at medical facilities in the city. If there is an influx of inquiries from people who have yet to receive vaccination tickets, it will hinder regular medical care services.

Please wait to receive your vaccination tickets before making inquiries. Your understanding and cooperation are greatly appreciated.

If you are currently receiving outpatient treatment or have an underlying medical condition, please first consult with your doctor regarding the vaccine.

Q&A regarding the vaccinations

Q: Why do I need an appointment?

A: Appointments are required to use limited amount of vaccine doses without waste. The Pfizer vaccine that is currently being supplied has a limited storage period, so there is only a short amount of time when the vaccine can be used. Therefore, the vaccine is being supplied in stages, and vaccinations must be provided only by appointment.

Q: Can the vaccine be provided to everyone who wishes to receive it?

A: Preparations are being made to ensure that the vaccine is provided to everyone who wishes to receive it.

Q: Can I receive a vaccination at a medical facility outside the city?

A: As a rule, vaccinations should be received at medical facilities located in the municipality of your resident registration. However, those with underlying medical conditions can receive the vaccine through their regular physician, and those with unavoidable circumstances will be able to receive the vaccine at medical facilities outside the city. An application may be required, so please inquire with the municipality of the medical facility where you wish to receive the vaccine.

Please help the vaccinations go smoothly by being well-prepared

The proper preparations and checks done by each person can help the vaccination process go more smoothly as the local medical facilities begin to provide individual vaccinations. Be prepared before your vaccination by filling out your “Prevaccination Screening Questionnaire for COVID-19 Vaccine” (予診票) ahead of time and wearing appropriate clothing to help reduce the burden on medical facilities.

Appropriate clothing for the day of your vaccination

The vaccination will be injected into the muscle of the upper arm near the shoulder. In order to receive the vaccination smoothly, please wear a shirt that lets you expose your shoulder easily when you remove your jacket.
