Changes to the payment method and copayment amount for single-parent household medical expenses

Starting on Sunday, November 1, members of single-parent households will be able to pay the copayment amount on the insurance coverage chart by presenting the Single-Parent Household Medical Expense Subsidy Ticket when obtaining treatment at a medical facility in Chiba Prefecture. Medical Expense Subsidy Tickets will be sent by mail to eligible households, starting on Wednesday, October 21. Along with the Medical Expense Subsidy Ticket, you will find a return envelope for the Children’s Medical Expense Subsidy Ticket. When you receive the Single-Parent Household Medical Expense Subsidy Ticket, please put the Children’s Medical Expense Subsidy Ticket that you have been using in the envelope provided and return it by mail.

Regarding Special Fixed-Amount Benefit Payments for Newborns in Kashiwa City

In Kashiwa City, newborns born on or after April 28, 2020, who were ineligible for the Special Fixed-Amount Benefit Payment will now be provided with 100,000 yen. For each eligible household, an application form will be sent to the head of household starting at the end of October.
Eligibility
Head members of households meeting all of the following criteria completely.
- A newborn born between April 28, 2020 and April 1 of next year belongs to the household.
- A newborn baby is registered in the Basic Resident Register of Kashiwa City because of his or her birth.
- The same benefit payment is not received in another municipality.
Application deadline
Monday, May 31 of next year.
Benefit payment amount
100,000 yen for each newborn.
Application
Complete the required parts of the application form, collect the required documents, and send the application form and documents by post in the return envelope provided. (Applications received will be valid according to the postmarked date.)

By Tuesday, December 1, please complete all required parts of the application form, and send it to postal code 277-0005, Kashiwa 1-chome 7-1-301, Kashiwa City Cross-Cultural Center. (Applications received will be valid according to the postmarked date.) Applications can also be submitted by email (boshu-1@kira-kira.jp) or in person. Application forms are available at the Cross-Cultural Center.

- Participants will be selected by draw if there are more applications than spaces available. Participants will be notified by Friday, December 18.
