The new "My Number" Individual Number system (social security and taxation number) will begin, making everyday administration more convenient. We'll explain in simple terms what kind of system it is, and where you will be required to use it.
What is the "My Number" Individual Number system?
This system will provide each individual resident living in Japan with a unique 12-digit number. It will also be sent to babies who have just been born, and foreign residents in Japan with a Resident's Card. The "My Number" Individual Number will be used for one's lifetime, and as a rule, cannot be changed.
Delivery in November
In this city, the "Notice Card" with the 12-digit "My Number" Individual Number written on it will be delivered by registered mail to the current address (as of Monday, October 5) on your Resident's Card.
The Notice Card is a paper card without a facial ID photo. It includes the "My Number" Individual Number, your name, address, date of birth, etc. Please make sure to keep the card in a safe place.

※Please be careful of unauthorized canvassers taking advantage of the "My Number" individual number system to obtain personal information. Please be aware that there will be no request to submit any forms to government organizations before the notice cards are delivered.
Where can it be used?
Starting from January of next year, the use of the individual number will be progressively introduced in places such as national government facilities and local public organizations in areas such as social security, taxation and disaster measures.
●Various types of welfare-related application processes such as child allowance, etc.
●Documents submitted to the National Tax Office, such as tax returns, statutory reports, etc.
●Tax and social security-related application processes for commercial business.
In order to prevent identity fraud, a strict confirmation of personal identity (identity confirmation and number confirmation) will be done when these kinds of administrative processes take place.
In the future
Starting from July 2017, an information link is scheduled to begin between national government facilities and local public organizations. In addition to reducing the number of visits to reception desks for public administrative processes, it will also simplify the forms that need to be submitted.

What is an Individual Number Card?
The Individual Number Card is an official identification certificate that lets you confirm your photo identification and your individual number in one card.
The card is plastic, and has a smart card IC chip. The front of the card lists your name, address, date of birth, gender, and includes your photo ID. Your "My Number" individual number is included on the back of the card. This card can be used for the submission of forms that include your "My Number" individual number, and in other places that require personal identification.
Benefits of the card
● It lets you confirm your photo identification and your individual number in one card.
● You can submit electronic forms such as e-Tax using the electronic identification recorded in the smart card IC chip.
● You will be able to obtain documents such as your resident's certificate from convenience stores, etc. ※This service is scheduled to begin in Kashiwa City in October of next year.

How to apply
You may choose from two application methods for your Individual Number Card, by post or online. The first issuance of the Individual Number Card is free of charge.
In November, the "My Number" individual number Notice Card will be delivered by registered mail to the address on your Resident's Card.
Please complete the required parts of the application form included with the Notice Card, attach an ID photo, and return it by mail using the return envelope provided.
From January of next year, you will receive a card issuance and notice set (a 3-item package with the notice of issuance, the notice card return form, and an information pamphlet). Please confirm the information included in the package, and make an appointment to receive your card.
Please come to the reception desk in the City Hall, main building on the 1st floor at the date and time of your appointment. Bring the required documents, and make sure the applicant comes in person.

Q & A
What will happen with my Basic Resident Register Card?
The card can be used until the expiration date, however starting from December, it will no longer be newly issued. In addition, since you cannot hold both an Individual Number Card and a Basic Resident Register Card, the Basic Resident Register Card will have to be returned when you apply for an Individual Number Card.
What will happen with the electronic certificate (official individual identification certificate) registered with the Basic Resident Register Card?
This can be used until the the expiration date.
If you are planning to submit your tax return next year by e-Tax, and your Basic Resident Register Card expires before the tax return is scheduled, please renew your card by November. After January of next year, you will need to replace the card with an Individual Number Card.
Will I still be able to use the card reader that was used for the Basic Resident Register Card?
The reader that was used for this year's tax return will also be able to be used with the Individual Number Card.
I'm concerned about personal information being leaked...
The use of the "My Number" individual number is limited by law, and the organizations and situations where it is handled are clearly described by law. The management situation is monitored by a 3rd-party organization, and strict penalties will be enforced if the law has been broken. In addition, the Individual Number Card has an ID photo, a set password, and the smart card IC chip on the back of the card does not have highly sensitive private information such as tax information recorded in it.
What is the expiration date of the Individual Number Card?
For those who are over 20 years of age, the card will expire on their 10th birthday after the card has been issued. For those who are 19 years of age and under, it will expire on their 5th birthday after the card has been issued.
What if there are changes to the address or name on the Notice Card or Individual Number Card?
When you register a change of address or a marriage, you will need to present your Notice Card or Individual Number Card. At that time, the information on the card will be changed.

Please answer the census survey according to your situation as of October 1st.
