List of Group Vaccination Covid-19 Vaccine, Booking required

Group Vaccination of Omicron compatible vaccine is available during January at the venues in the following list.　At the Chuo taiikukan, Omicron compatible Vaccine (BA.4-5) from Moderna is available from 14th (Sat) January. Please make a booking online or by calling COVID-19 Vaccine Call Center, Kashiwa city (Tel: 7179−2663・7179−2313).

Eligibility

Anybody of 12 years old and older with registered address in Kashiwa and 2 or more vaccinations done.

What to bring

Ticket with pre-vaccination screening questionnaire, self-identification document, prescription record book　(if you have one)

 

You can purchase the Antigen Test Kit at the price of 500 yen (by 31st January)

500 yen per kit ※ only one per person

You can purchase for the whole members of the same household at one purchase

Confirmation documents of all the eligible person

When tested positive, you have to register yourself to the Person tested positive for COVID-19 registration center (HER-SYS), Chiba Prefecture
