Reopening of Recovery Accommodation facility

　Due to the spread of COVID-19, we will reopen the Recovery Accommodation facility and start again to offer the accommodation to COVID-19 patients with mild symptoms. The operation of the facility is conducted by the subcontractor under the supervision of Kashiwa city, with the advice from the specialists. We would appreciate your understanding.

Health condition monitoring by nurse and other staff, Foods and daily supplies   ※staff wear protection suits when carrying out their duties in infected area.

 

Vaccination certificate will be available at  Seven-Eleven convenience stores from 17th August (Wed)

Anybody who owns “My number” card can request the issue of the Vaccination Certificate at Seven Eleven convenience stores. Vaccination Certificate is to officially certify the type of the Vaccine and the dates of the Vaccination. *Those who have not hold “My Number” card yet can request by post.

Those who have been issued Vaccine Certificate for foreign travel once after 21st July at Health Promotion Division (3rd floor, Wellness Kashiwa) or on COVID-19 Vaccine Certificate App can request the issue of Vaccine certificate for foreign travel at Seven Eleven convenience stores.

Make sure to collect new card of National Health Insurance・Elderly Medical Treatment Insurance

　Returned Health Insurance cards which had been sent out in July are now kept at Health Insurance and Pension Division. If you haven’t received your new insurance card yet, please make sure to collect it at the earliest stage.

How to collect

 ▶Desk＝Collect from Health Insurance and Pension Division (1st floor, City hall) with Official Self ID with photo  ▶Request to be mailed by post＝please contact Health Insurance and Pension Division

If you have registered to any other health insurance scheme, do not forget to withdraw from National Health Insurance.

Please submit Income statement for Special child support allowance

Anybody on Special child support allowance scheme has to submit the Income statement and allowance certificate by 12th September (Mon) to Disability Welfare Division (2nd floor, City hall Annex) or Shonan branch (1st floor, Shonan building). Once you miss the closing date, you will not able to receive the allowence on time.
