Extending the operating period of Vaccination center at night

Operating period of “Vaccination center for COVID-19 at night”located in Emergency medical clinic at night is extened till 4th June (Sat). Due to this extention, Emergency medical clinic will be closed till 5thJun(Sun). Please be aware of the changes.

 

Time	Mon〜Sat 1900PM～2100PM※Except National holidays. Reception closes at 2000PM
Location	Emergency medical clinic at night （WELNESS kashiwa）
Eligibility	Over 12-years-old holding Vaccine ticket for 3rd shots with address registered in Kashiwa
Capacity	48 slots per day
Vaccine	Pfizer
Booking	Every Wednesday 0030PM, Booking for the week after the next.

Kashiwa-city has set “drop-in Vaccination slot for those who has difficult to plan and book the date for the vaccination.

Requirement	Anybody who fill all the following requirements ▶over 18-years-old on the day of vaccination ▶bring the Vaccine Ticket for the 3rd shots▶passed 6-months after the 2nd vaccine shots
What to bring	Vaccine ticket, Prevaccination Screening Questionare slip, Identification document, Document of the record of 2nd Vaccine shots
