Applications by mail are now being accepted for the Special Fixed-Amount Benefit Payment

To implement the national government’s "Emergency Economic Measures Due to the Spread of COVID-19” in Kashiwa City, we have begun to accept applications by mail for the Special Fixed Amount Benefit Payment. The application forms will be sent out on Monday, May 25, addressed to the head of each household. Please check that you have received yours.
Points to be careful of when making your application by mail
- The parts of the form that are within a bold outline are all completed.
- Your bank account information is entered correctly. Check that the name and code of your financial institution, and the name and code of the branch are all correct.
- Make sure a copy of your personal identification documents are attached.
- Make sure a copy of the documents to confirm your bank account are attached.
- Remove the notice section at the cutting line, and keep it for your records until you have confirmed that the benefit payment has been deposited.
- Mail the application form.
Applications cannot be submitted directly at the city hall.
There is a deadline for the application
The deadline for the application is Monday, August 31, 2020 (applications are considered valid based on the postmarked date). Please be aware that if there is no application by the deadline, it will be considered to be declined, and you will not be able to receive your benefit payment.
What should I do in these kinds of situations?!
I do not understand how to fill in some parts of the form.
Please contact the Kashiwa City Benefit Payment Call Center. To prevent the spread of COVID-19, there is no face-to-face advisory counter. Please refrain from coming to the city hall for consultation.
I made a mistake on the application form!
If you make a mistake when you are filling out the information, cross out the mistake with a double line, use a revision seal (tei-sei-in), and write the correct information in the margin.
I don’t know which part of my bank book to copy as part of my bank account information document.
It will be necessary to check the information written in the application form against your document. Please make a copy of the part of your bank book which shows all the information that you wrote in the application (the name of your bank, branch name, account number, and the account holder’s name in furigana). Depending on the type of bank book, you may need to make copies of more than one page.
I live in Kashiwa City, but I have not received my application form yet.
Application forms will be sent out to addresses listed in the Basic Residents’ Register as of April 27. If you have not received your application form by Wednesday, June 3, please contact the Kashiwa City Benefit Payment Call Center.
I would like to know the status of the receipt of applications, and when the payment will be deposited.
Application inspections are being processed in order. However, an extremely large number of applications are being processed, so the acceptance status of individual applications cannot be provided. You will be contacted if there is a problem with your application.
Online applications are now being accepted
Those who have a My Number Card and a card reader can make use of the “My-Na Portal” online service to make their application.
Some people who have made an online application may still receive an application form by mail. However, there is no need to mail the application.

Your understanding and cooperation is appreciated
In Kashiwa City, it is expected to take 2 to 3 weeks before benefit payments are deposited. A large number of applications are expected immediately after the reception service begins, so application processing may take longer than normal. In addition, confirming information on incomplete or incorrect applications will take more time, resulting in an even longer delay for the payment. When you are filling out your application, please check the information one more time before sending it, and ensure that there are no errors or omissions. We will provide financial support for small and medium-sized enterprises, and individual business operators who are experiencing a large decline in sales due to the effects of COVID-19. If the financial benefit and/or support criteria is met, combined benefits can be received.

Fill in the required parts of the forms, attach the necessary documents, and send them by mail to postal code 277-8505, Kashiwa City Hall, Kashiwa City Financial Support Benefits for Small and Medium-Sized Enterprises. (The postmarked date is valid for applications received.)
